To pre-qualify and speed up the process for your Contractors License Bond, submit your application online. Submission of your application for a Contractors License Bond electronically requires your authorization to obtain both individual and/or business credit reports on all owners, officers, partners, LLC members and spouses. Once pre-qualified, your bond(s) will be issued to you after we receive your completed and signed application along with payment of your first year’s premium.

1.    By submitting your application online, principals, indemnitors, applicants and their spouses hereby expressly authorize CB&MS or applicable underwriting company to access credit records and to make such pertinent inquiries as may be necessary from third parties sources for underwriting purposes.

2.    Ensure that all questions are answered and that area codes are included for all phone numbers. Incomplete applications may delay the processing of your application.

3.    Sign your completed and printed application on behalf of the company. Each owner, partner, stockholder or LLC member and their spouses sign the Indemnity Section under “Individual Signature”. If more than two (2) Indemnitors, print another application and have the third (3rd) and any additional person(s) complete and sign the Indemnitor Section.

4.    Mail your completed application to CB&MS along with a check for payment of the License Bond requested:
